Georgia THRIVe Prevention (GTP)


Georgia THRIVe Pathways is a comprehensive triage, clinical support, and engagement program designed to assist families facing substance use disorder and opioid challenges. Building on the existing Georgia THRIVe model, the program offers personalized care plans, including opioid use disorder screening, linkage to treatment, enhanced home visiting, educational resources, and peer support.

The mission of Georgia THRIVe Pathways is to empower families affected by opioid use disorder through accessible, individualized support and treatment, fostering safe, healthy environments for children and strengthening communities across Georgia.

The program’s vision is to establish a coordinated system of services that provides trauma-informed, culturally responsive support to families affected by opioid use, resulting in safe, nurturing environments where children thrive and communities are strengthened.

Georgia THRIVe Pathways is a program designed to support families impacted by substance use disorder and opioid challenges. It provides personalized care through screening; connecting individuals and families to treatment services; and offering enhanced home visiting programs, educational resources, and peer support. Its goal is to help families navigate recovery, improve health outcomes, and build stronger, safer communities.

  • Who is Eligible?

 Pregnant OR parenting a child 0-3 years of age

AND

 Shows early warning signs of possible mental health, emotional, behavioral, substance use, or opioid use problems.

Or a family member or loved one of the above
